You need to fill in this form and send it back to the Nomi city hall.
This is to allow a student (no tax, no income) to have pension exemption.
The envelop looks like the below picture:
Fill in the form:
1st row:
- 1st column: JAIST
- 2nd column: JAIST address
2nd row:
- 1st column: valid date in your residence card (from -> to).
Remember to write in Heisei year (e.g., 2015 = 27)
- 2nd column: date you wish to have pension exemption
3rd row:
- 1st column: do you have income (1: yes, 2:no)
- 2nd column: do you pay tax (1: yes, 2:no)
-> choose corresponding numbers
4th row:
- your address:
- name
- hanko
- phone number
(If you have mistake some place, just put your hanko in that place.)
After filling in this form, stick the blue paper in this side form
Write your name and address in the back side of the form
Tear the papers a half, and only send the filled form (the left side)
Send it by post office
